Lower Eyelid Blepharoplasty Explained: Techniques, Terms & Modern Approaches

Lower eyelid surgery (lower blepharoplasty) is one of the most effective ways to restore a refreshed, well-rested appearance. But if you’ve started researching your options, you’ve likely come across a variety of technical terms that can feel overwhelming.

Understanding these concepts can help you feel more confident and informed as you consider treatment.

What Is Lower Eyelid Blepharoplasty?

Lower eyelid blepharoplasty is a surgical procedure designed to improve the contour of the lower eyelids. It commonly addresses:

  • Under-eye puffiness
  • Hollowing or volume loss
  • Loose or excess skin

The goal is not to change how you look but to restore a smoother, more natural transition between the lower eyelid and cheek.

Surgical Approaches: How the Procedure Is Performed

There are two primary approaches to accessing the lower eyelid:

Transconjunctival approach involves making an incision on the inside of the lower eyelid (the conjunctiva). Because the incision is hidden, there is no visible external scar. This approach is often preferred when addressing fat repositioning or removal without needing to remove excess skin.

Transcutaneous approach uses an incision just beneath the eyelashes. It allows for direct access to both fat and skin. While effective, this method can carry a higher risk of lower eyelid retraction in certain patients due to disruption of supporting structures.

A skin pinch is a technique used to remove a small amount of excess lower eyelid skin through a subtle incision beneath the lashes. Although it does create a scar, it typically heals very well and is often barely perceptible.

Understanding Fat Management in Blepharoplasty

One of the most important decisions in lower eyelid surgery is how to manage fat. This is where surgical artistry plays a major role.

Subtractive blepharoplasty is when fat is only removed (not repositioned) to reduce under-eye puffiness and best suited for prominent under-eye bags and minimal hollowing.

Fat transposition is when fat is repositioned to smooth the transition between the lower eyelid and cheek and best suited for puffiness combined with hollowing. This technique is used when patients are seeking a seamless, natural contour.

Fat transfer (Fat Grafting) is when fat is harvested from another area of the body (commonly the abdomen or thigh) and used to restore lost volume in the lower eyelid and cheek. This technique is best suited for patients with significant volume loss presenting with a “deflated” appearance under the eyes.

Supporting Procedures That Enhance Results

In some cases, additional techniques are used to support eyelid position and optimize healing:

Canthopexy is a supportive suture placed to stabilize the lower eyelid without cutting the tendon. This can reduce the risk of postoperative retraction.

Canthoplasty is a more advanced tightening procedure where the lower eyelid tendon is shortened and reinforced to improve eyelid shape and support.  This technique is best suited for patients with significant lower eyelid laxity or sagging.

Temporary tarsorrhaphy is a stitch that gently supports the eyelid during early healing, helping maintain proper position and contour.

Laser Resurfacing: Improving Skin Quality

Laser resurfacing (CO2 or Erbium) is often combined with lower eyelid surgery to:

  • Smooth fine lines
  • Improve skin texture
  • Stimulate collagen production

This enhances the overall rejuvenation of the lower eyelid and surrounding skin.

Not One-Size-Fits-All: How Surgical Plans Are Personalized

Every patient’s anatomy is different, which means the best approach is always customized.

A simplified way to think about treatment planning:

  • Fat removal → best for puffiness without hollowing
  • Fat transposition → ideal for puffiness + hollowing together
  • Fat transfer → used when volume loss is the primary concern or a significant component.

Many patients benefit from a combination approach to achieve the most natural, balanced result.

The Goal: you, just refreshed. The most successful lower eyelid surgery doesn’t look like surgery at all.

It looks like:

  • You slept better
  • You feel more energized
  • You look like yourself, just refreshed
